Plastic surgeons recently less and less operate, adopting methods of cosmetologists. A famous plastic surgeon in Russia, Sergei Prokudin, goes to Prague in early November to conduct an author's master class on how to correct age-related changes and improve the appearance without resorting to surgery.
Your author’s master class is called “Three whales of non-surgical face beautification”. From the surgeon to hear the word “non-invasive” unexpectedly. Did you like to operate?
In modern plastic surgery, this is not unexpected. According to statistics, leading American surgeons make 40% of manipulations today without a scalpel, using fillers and botulinum toxins. Now there are many things that, in principle, do not need to operate. If done correctly, non-surgical techniques give good results. In addition, surgeons can still go a little further than cosmetologists. For example, the method of volumization, which is so popular today in Europe, is when the volumes lost with age are replenished with the help of special preparations for sculptural modeling of the face - volumizers. So this is a surgical invention.Perhaps, plastic surgeons are simply taking over from their patients, who prefer less traumatic methods?
We simply try to achieve an optimal result, following the path of least resistance. Various techniques that are now in service with plastic surgeons are just tools. You set the task and choose the tool. To any place can be reached in different ways. And you always need to go the shortest way. Life shows that the shortest path is the simplest and most productive. You know, Pirogov (Nikolai Pirogov is a surgeon and anatomist) said that if you heard the clatter of hoofs outside the window, you will not see a zebra there, there will be a horse.What can be changed on the face without resorting to surgery?
Everything that is connected with volume changes on the face today can be made by fillers. Filler very much replaces prosthetics, for example, chin, zygomatic and temporal zones. When I began to use fillers in the front middle third of the face, the operations in this zone practically ceased to be done. Because fillers are more effective with less injury. There are a large number of techniques where it is easier to do with fillers. Yes, it will not be so long-term. On the other hand, the validity period of a good volumer is from one and a half to two years with a good bolus technique (when the drug is placed very deeply on the periosteum - ed.).What beauticians may be interested in your course?
I can teach a lot. I was lucky, I communicate with almost all world anatomists, such as Brian Mendelssohn, Yves Saban, Dr. Ali Piraesh, with a number of American doctors who today form the view on anatomy in principle and on the anatomy of aging of the face of the neck, in particular. Everything that is further discussed at all world congresses on aesthetic medicine, in my course, you can learn first-hand in plain Russian. The second is the practical application of this anatomy to the main cosmetology techniques. I, above all, hard practice, I operate a lot. Now the 21st year has gone, as I do.
And although the main cosmetological methods, such as fillers, botulinum toxins and threads, are known, almost every year new concepts appear that allow you to give the patient what you need more safely, effectively and prolongedly. And finally, in my course I always pay a lot of attention to safety. Security is one of the biggest problems of the modern cosmetology world. By conducting a large number of anatomical courses, I see that there are still a large number of zones that are not well understood. And if you explain them, you can avoid a large number of complications, such as ischemia, hypercorrection, damage to the nerves and blood vessels. I have quite clear and verified information about how it all works, because all the causal relationships still lie in the anatomy.What is the need to hold these courses in Prague?
The dissection of the anatomical course, organized by Medical Esthetic, is held at the Anatomical Institute of Charles University. In Russia, unfortunately, there is a certain number of laws that limit the ability to work on biological material. This is a problem, modern students learn more on dummies than on corpses. A normal surgeon will work out any new method on cadaveric material. Practice simulation is the only way to answer the questions that arise when a patient comes to you. And at Charles University there is such an opportunity, there are sectional rooms where students work. Anatomy today can answer almost all questions. And my task is simple, after such courses cosmetologists easier prick, begin to work without fear and doubt. Because doubts give rise to not very good results.Why should surgeons participate in dissection? Do they still have any questions about anatomy?
This is interesting, itchy naturalist, if you will. Each dissection hides something new, even for a surgeon. A number of concepts, which I will publish, were born in the dissecting room. And the people I work with leave at 10 o'clock in the evening just because they are interested. There are such crazy people, but they move science.In which country is the most interesting in the industry? Where do all plastic surgeons go?
Definitely in the USA. America has long dictated fashion to us, because the basic techniques come from there. Practically everything new that is happening in the world of plastic surgery is compiled there. For example, the American magazine Plastic Reconstruction Surgeons. Printed there for a professional - happiness. Of course, there are French, Italian and other schools, but America was originally a pioneer in the industry. Then the Americans accumulate a large number of nationalities, and as a result, they offer more universal methods.
